- Over the telephone e.g. to confirm details of your personal information/ treatment status/ referral source/ appointment time and date etc

- Through the website e.g. if you contact us via our ‘contact us’ page with an enquiry
- On initial assessment when you complete Consent Forms
- At the point of referral from an external source e.g. through an insurance intermediary that may be referring you

Contact details submitted through the contact form and enquiry emails will be destroyed after either
a) the end of our correspondence, or
b) the completion of a consent form at your initial appointment
Information from your consent form and medical records is kept 6 years after your last treatment or 3 years after death. For anyone under the age of 18 records will be kept until their 26th birthday or 3 years after death.
The basis on this we process your sensitive information (special category) is so that we can make recommendations on a course of treatment and rehabilitation

You agree that you will indemnify us against any and all loss or damage suffered, including any legal fees or costs, as a result of any breach of this agreement by you.
